The Clinical Care Department manages patient referrals, initiated from the primary care provider within the Spirit Lake Health Center, to outside specialist(s). The process allows for a smooth transition of care to specialty care. Clinical Care provides continuity of care to meet the specific needs of the patient; the goal is to improve patient satisfaction, experience, as well as enhance patient quality of life. In addition, we provide patient centered care through an interdisciplinary approach essentially through “building a bridge” of communication with specialty care providers. Care is managed with the patient, local providers, internal and external healthcare facilities across North Dakota and surrounding areas. We also connect the patient with resources and services available; such as medical services/support, benefits, transportation arrangements, rooming, etc.
Services We Provide
- Managing referrals to/from outside vendors for specialty care and continuity of care
- Assists the patient with scheduling/rescheduling appointments regarding the referral to outside vendors
- Fax appointment reminders to Dakota Health and Wellness for transportation/fuels assistance/lodging for scheduled appointment visits
- Refers patients who lack insurance to apply with our in-house Benefits Coordinators to better manage referrals to specialty care
- Courtesy calls to patients after an ED visit or inpatient stay
- Inquiries about scheduling a post-hospital follow-up appointment with a Primary Care Provider at Spirit Lake Health Center
- Updates referrals appointment dates and times
- Serves as a patient advocate
